Capricorn Group Care is an RN-guided, non-medical senior placement, provider marketplace and care-transition coordination platform. We help families and healthcare professionals discover and connect with senior-care providers and transition-service partners while supporting a more organized journey from initial inquiry through placement and transition.

Independent Providers and Partners

Participating providers and transition-service partners are independent organizations responsible for their own licensing or lawful operating status, staffing, services, pricing, assessments, admissions and regulatory compliance. Capricorn is not the licensing authority for participating providers, is not the employer of independent marketplace providers, and does not make the final decision regarding provider admission.

Family Choice

Capricorn may help families identify and compare relevant options, but the family or authorized decision-maker makes the final selection. Provider pricing, admission requirements and service agreements remain between the family or resident and the selected provider.

Complimentary Family Support

Families generally do not pay Capricorn for standard placement-navigation assistance. Capricorn may receive compensation from a participating provider following a successful placement or under a separate marketplace agreement. Families are not required to select a participating provider and retain control over their final decision.

No Guarantee

Application, onboarding, verification or publication of a provider or partner profile does not guarantee inquiries, referrals, placements, admissions, availability or outcomes.
