PLACEMENT AND TRANSITION COORDINATION
Support family communication, placement-navigation workflows, provider outreach, tour coordination and transition follow-up.
PROVIDER ONBOARDING AND MARKETPLACE READINESS
Help independent providers complete marketplace profiles, submit required information, respond to correction requests and maintain accurate service and availability information.
HEALTHCARE AND COMMUNITY PARTNERSHIPS
Develop relationships with hospitals, rehabilitation facilities, social workers, case managers, physicians, hospices, community organizations and senior-resource networks.
MARKETPLACE OPERATIONS AND TECHNOLOGY
Support provider data management, workflow administration, quality assurance, digital content, search visibility and platform operations.
COMMUNITY CARE AMBASSADOR OPPORTUNITIES
Help introduce families, providers and community organizations to Capricorn’s placement-navigation and marketplace pathways.
